Incoming 7 th and 8 th grade students interested in trying out for fall sports must be registered in FamilyID and have a valid physical on file with the nurse. Information pertaining to registration is available on the athletic website.Īnnual Screenings: Screenings are conducted for height, weight, blood pressure, vision, hearing and scoliosis according to a schedule set by the State of New Jersey.Īdditional pertinent information such as: Attendance, early dismissal, working papers and cafeteria regulations may be obtained from the student handbook.FamilyID is now open for fall athletic registration. Physical Education Excuses, Crutches and Elevator Passes: Students that require an excuse from physical education (gym) or an elevator pass must have a note from the physician/health care provider for crutches, orthopedic braces or any other medical appliance.Īthletes: Sports registration is required every sport season. Medication orders: In order for a nurse to administer medication there must be an administration of medication form completed by the physician, signed by the parent, submitted to the school nurse with the medication in its prescribed container. The nurse does not diagnose illness, prescribe treatment or dispense medication of any type including over the counter drugs without a doctor’s order. The Monroe Township Schools are staffed by a qualified school nurse. The nurse’s primary objective is to provide emergency first aid in case of an accident or sudden illness.
